There are two ways to enroll your child at a Kidango center.- By Phone
Enrolling your child at Kidango starts with one phone call to 1-800-262-4252. Our helpful staff will take your mailing information and send you an application for child care. Please return the application promptly so we can place your child in the appropriate center.
- Online
Click this link and use our online form to submit your information. Your information will be added to our secure database and you will be contacted when we have openings at the centers you choose on your application.
The following documentation is required prior to coming to your enrollment appointment or an open enrollment:
- Child's Birth Certificate
- Child's Immunization Record
- Physician's Report (due 30 days after start date)
- Child's Preadmission Health History
- Identification and Emergency Information
- Consent for Emergency Medical Treatment
- Emergency Identification Information (English)
- Infant/Toddler Needs and Services Plan (for children 0-24 months)
Please bring the following if requesting financial assistance:
- Birth Certificate of all children included in the family size under 18 years old
- If employed, one month of current paystubs
- If unemployed, documentation of one month of current income
- If self employed, business license, income tax return, documentation of days/hours of operation
In addition, for the full-day program, please complete the following items:
- If employed - Employment Verification
- If incapacitated - Incapacitation Form
- If in training - Training Verification Form and a printout of your school schedule
Optional forms as needed:
- Medical Statement to Request Special Meals/Accomodations (optional)
- Medication Administration Form (optional)
- Vegetarian Request Form
